Dr. Nightshade uses the antidote on herself and John, but before she can administer it to Capwolf, an evil Captain America doppelganger teleports in and attacks. Stabbed with the syringe in the confusion, Capwolf begins reverting to human form during the fight. Dr. Druid senses the doppelganger isn't really alive, so the now human Captain America impales the double on its own bladed shield.
